When do you apply for a duplicate?
Has your residence or establishment permit or declaration been stolen, misplaced, lost or burned? Then you can apply for a duplicate. The VZ document list describes which supporting documents are required.

What do you need?
- Proof of the valid residence or establishment permit or declaration showing that residence was granted
- Request form with only the names of the persons appearing on the lost document
- Passport: holder page and the pages in which the relevant residence was stamped
- CBB registration declaration, not older than six months
- Police declaration in case of theft, loss, disappearance or burning

What happens after submission?
According to the old VZ instructions, you will immediately see a registration confirmation on screen, receive an e-mail within 24 hours and be asked to respond to that confirmation e-mail within 48 hours. To be confirmed by VZ
